Tarakis was an uninhabited M-class planet in the Delta Quadrant.

The planet was once home to a Nakan colony. In the 21st century, a group of soldiers led by Commander Saavdra was ordered to relocate the colonists, supposedly temporarily. However, the colonists resisted with arms, and the situation devolved into the Nakan massacre, in which 82 Nakan were killed.

Following the incident, a memorial monument was erected on the site of the massacre. The monument contained a synaptic transmitter, which emitted a neurogenic pulse that placed memory engrams from the soldiers involved in the massacre in the minds of any individuals passing through the Tarakis system. The monument was abandoned in the 22nd century, and by the 24th century its power cells began to fail, causing the memories to become disjointed.

In 2376, Chakotay, Tom Paris, Harry Kim, and Neelix scouted Tarakis for dilithium on the Delta Flyer. Upon their return to USS Voyager, they began to experience vivid memories of the Nakan massacre. After tracing the cause to Tarakis, Captain Kathryn Janeway ordered that the monument be repaired and that a warning buoy be placed in orbit, so that future passers-by would know what to expect. (VOY: "Memorial")

Inscription at the base of the memorial (translated):
Words alone cannot convey the suffering.
Words alone cannot prevent what happened here from happening again.
Beyond words lies experience.
Beyond experience lies the truth.
Make this truth your own.
