(covers information from several alternate timelines)

Jerry Fleck was a Starfleet officer in the 24th century.

In the late 2360s, he was a captain at Starfleet Academy. He was assigned as a Flight Ops officer. His name, rank, and title were listed on the Flight Proficiency Award for Cadet Harry Kim. (VOY: "Non Sequitur", set artwork [1])

In 2371, he was listed as a Fleet Ops officer on the dedication plaque of the USS Voyager. (VOY: "Caretaker", dedication plaque)

In 2372, he was listed as a Starfleet Command officer on the dedication plaque of the USS Enterprise-E. (Star Trek: First Contact, dedication plaque)

In 2374, he was listed as a Fleet Ops officer on the dedication plaque of the USS Prometheus. (VOY: "Message in a Bottle", dedication plaque)

In an unknown, alternate future timeframe, he was listed as a Tactical Systems officer on the dedication plaque of the USS Pasteur. (TNG: "All Good Things...", dedication plaque)

Fleck was named after first assistant director Jerry Fleck.
This character might be the same as the Jerry Fleck mentioned in Joseph Carey's Starfleet Academy degree. [2]
According to the text commentary for "These Are the Voyages..." by Michael and Denise Okuda, there was a Lieutenant Commander Jerry Fleck aboard the USS Enterprise-D in 2370, and he was assigned living quarters across the hall from Counselor Deanna Troi. The character's name was placed on the door across the hall from Troi's quarters on the rebuilt Enterprise sets. However the label was never visible on screen in the episode.
