It is unknown if there is an etymological or mythological relationship between the Fek'Ihri and Fek'lhr, the hideous guardian of the gates of Gre'thor.

In the storyline of Star Trek Online, the Fek'Ihri were the footsoldiers of Molor during his rule of Qo'noS, and banished with him to Gre'thor; they seem to be similar to Fek'lhr in appearance. Klingon players who reach the rank of captain must fight off a Fek'Ihri invasion of the Klingon Empire. It is hinted, though not explicitly stated, that they may have been artificially created by the Hur'q. In the expansion "Victory is Life", it is revealed that they were in fact created by the Dominion, as precursors of sorts to the Jem'Hadar (and similarly engineered with ketracel-white addiction), their appearance based on Klingon folklore to inspire fear. However, the Fek'Ihri broke from their masters and allied with Molor, before descending into madness and becoming the very monsters they resembled.
