The Commander, Starfleet was a high-level Starfleet position given to a Fleet Admiral. It is similar to the Starfleet Chief of Staff and is only subordinate to the Commander in chief. Fleet Admiral Harry Morrow held the position in 2285. (Star Trek III: The Search for Spock) Fleet Admiral Cartwright took over from Admiral Morrow in 2286 and held the position until 2293.
